This informed consent form is for adult, female participants with experience in dating and romantic relationships who we are inviting to participate in our research project titled, “What Women Want Project”. Our group is seeking to research women’s dating experiences, preferences and romantic outlook.

This research would require you, should you feel comfortable, to complete either a questionnaire or physical interview with one of our trusted team members that will take about [TIME INTERVAL]. 

PARTICIPATION TERMS

Please note that: 

  1. You may discontinue your participation in the interview/questionnaire at any time with no adverse consequences or prejudice whatsoever. The research is only worthwhile if you are comfortable enough to share your honest experience. 

  2. You will remain fully anonymous throughout the course of processing and publishing of any date we collect during of our research. 

  3. The interviews may be recorded; however, these recordings are only to ensure that the data is correctly captured and will not be published. The recordings will be destroyed following the conclusion of the project. 

  4. You have the option of leaving your video on or off for the duration of the interview. You may freely request any further concessions to the structure of the interview. 

  5. You are free to halt any line of questioning you are not comfortable with, should you feel so inclined. 

  6. You are legally entitled to access, request the correction of, or the destruction of any records that contain your personal information. 

  7. The research project requires a minimum amount of information: 

    a. Identifying information: Names contact information, etc.

    b. Demographic information: Age, gender, cultural identification, relationship status, level of education etc.

    c. Personal opinions: views, preferences, etc. 

An example of a question may include: “What is your best and worst dating story?”
